For each site we read the public files every WordPress site serves (home page, feed, plugin readme.txt, theme style.css) and compare versions with WordPress.org, including plugins WordPress.org has closed, and why. Nothing is installed and no login is needed, so it doesn't depend on your management plugin staying connected.
What it can't see: plugins that load nothing on the home page, versions a site hides on purpose, premium plugins' latest versions. "Installed" comes from the plugin's own readme Stable tag, which nearly always matches the running version; treat a single finding as something to confirm in wp-admin.
